I am so glad you found your way here in your journey to health. 

To become a new patient: Just click the green "Book An Appointment" button in the top right of the webpage. There you can schedule a free 15min consult or a first office appointment that matches you or your loved ones needs.

First office visit: Typically we spend 90min together to review your health history, your current complaint or issue you need help with, and so I can really get to know YOU both physically and mentally.  I will do any pertinent physical exams and draw any labs that we may need as well. It is VERY helpful if you bring any recent lab work or records that relate to your case for me to review, and even more helpful if you send them over beforehand with your new patient paperwork. 

New patient paperwork: Its all online, super easy! Once you make an appointment in Jane, you will get a welcome e-mail that will prompt you to fill out the new patient forms. Please have them completed at least 24 hours before your appointment so we can make the best use of our time together. Set about 15-20min aside to do them as I ask some detailed questions. 

Confidential sharing of records and labs: The best way is to fax them to 720-588-2561. You can also include them in an e-mail attachment to info@drjessiemiller.com 

Insurance: I am an out-of-network provider for insurances in Colorado. This means that I cannot directly bill your insurance and do require payment (mastercard, visa, cash, check) at the time of the visit. I CAN take HSA cards as well and also provide a superbill if you would like to submit anything to your insurance. Many times labwork is covered and also some of my functional lab tests directly bill insurance and patients are having them covered. I do try to make it affordable for everyone, offer ½ off for kids under 12, and give discounts on any supplements you may need. I get it!
