New Patient Information
Welcome!
I am so glad you found your way here in your journey to health.
To Become A New Patient:
**Online booking (Dr. Miller only): Just click the green "Book An Appointment" button in the top right of the webpage. There you can schedule a free 15-minute consult or a first office appointment that matches you or your loved ones' needs.
**Nutrition by Kelsey: Please call or e-mail us to start the new patient process. We will then send you your intake forms, and once complete can book your first appointment. You can book a free 15-minute consult online at the green button above.
First office visit:
Typically, we spend 90 minutes together to review your health history, your current complaint or issue you need help with, and so I can really get to know YOU both physically and mentally. Dr. Miller will do any pertinent physical exams and draw any labs that we may need as well. It is VERY helpful if you bring any recent lab work or records that relate to your case for me to review, and even more helpful if you send them over beforehand with your new patient paperwork.
New patient paperwork:
It's all online, super easy! Once you make an appointment in Jane or over the phone, you will get a welcome e-mail that will prompt you to fill out the new patient forms. Please have them completed at least 48 hours before your appointment so we can make the best use of our time together. Set about 20-30min aside to do them as I ask some detailed questions. For Nutrition by Kelsey, once your forms are complete, including the 3 day food log, you will be able to book your first visit
Confidential sharing of records and labs:
The best way is to fax them to 720-588-2561. You can also include them in an e-mail attachment to info@drjessiemiller.com
Fees and Insurance:
Dr. Miller is an out-of-network provider for insurances in Colorado. This means that she cannot directly bill your insurance and does require payment (mastercard, visa, cash, check) at the time of the visit. We can take HSA cards as well and also provide a superbill. Labwork coverage is variable but we offer cheap cash based options, and some functional lab tests directly bill insurance. All our appt. fees can be found on our booking site here
FAQ:
Q: How long does it take to feel better.
A: Everyone has a different timeline based on their bodies response, how long it has been going on, and how well we are able to stick to the treatment plan. I like to say expect 3-6 months in working together to see results. We typically meet 1-2x/month.
Q: How much will this cost? Do you take insurance?

Q: Can you be my sole primary care provider?
A: The short answer is no. I do not have on-call or urgent care services or prescribe medication, making it not the best fit for PCP. I do however provide wellness visits that are individualized and in depth, including physical exam and routine testing, and am very passionate about preventative medicine. In conjugation with a primary care provider, NDs are a great choice for those seeking a more holistic view of their health with more treatment options beyond pharmacological medicine.
